Last week, The Moxie Health Group presented at the Post-Acute and Long-Term Care (PALTC) Medical Association Annual Conference (PALTmed) in Anaheim, marking its first time participating as a member of the organization.

From March 26 to 28, the team joined clinicians, medical directors, and healthcare leaders from across the country, all focused on improving outcomes in post-acute and long-term care. Over two days, Moxie shared its approach to care delivery and connected with professionals working across skilled nursing, assisted living, and related settings.

At the center of those conversations was one of healthcare’s most urgent challenges: hospital readmissions. Nationally, rates remain at 23.7%. At Moxie-supported facilities, that number is consistently reduced by more than half.

  • November 2025: 11.55%

  • December 2025: 12.74%

  • January 2026: 10.6%

These results reflect a model built on coordinated, physician-led care that prioritizes early intervention, real-time response, and alignment across care teams. Supporting that model is MoxieLink, the company’s proprietary, HIPAA-compliant platform designed to streamline care delivery and strengthen clinical decision-making.

MoxieLink unifies patient records, integrates AI-powered transcription, and automates coding validation, allowing providers to spend less time on documentation and more time focused on patient care. Its clinical-first design enables proactive workflows, including census screening to identify high-risk patients and ensure timely follow-up. The platform is now available on the PointClickCare Marketplace, expanding access to facilities nationwide.

Throughout the conference, the team engaged with peers across the industry, exchanged insights, and explored shared challenges facing post-acute care today. Conversations focused on rising patient acuity, staffing constraints, and the growing need for solutions that are both scalable and measurable.

The conference provided an opportunity to highlight what is working in practice while continuing to learn from others across the field. As the demands on post-acute care continue to grow, the focus remains on building models that deliver consistent, data-driven results.
