PALTmed Recap: Sharing a Model That Cuts Readmissions in Half

Last week, The Moxie Health Group presented at the Post-Acute and Long-Term Care (PALTC) Medical Association Annual Conference (PALTmed) in Anaheim, marking its first time participating as a member of the organization.

From March 26 to 28, the team joined clinicians, medical directors, and healthcare leaders from across the country, all focused on improving outcomes in post-acute and long-term care. Over two days, Moxie shared its approach to care delivery and connected with professionals working across skilled nursing, assisted living, and related settings.
At the center of those conversations was one of healthcare’s most urgent challenges: hospital readmissions. Nationally, rates remain at 23.7%. At Moxie-supported facilities, that number is consistently reduced by more than half.

January 2026: 10.6%
These results reflect a model built on coordinated, physician-led care that prioritizes early intervention, real-time response, and alignment across care teams. The conference provided an opportunity to highlight what is working in practice while continuing to learn from others across the field. As the demands on post-acute care continue to grow, the focus remains on building models that deliver consistent, data-driven results.
