Inside the Numbers Behind Fewer Hospital Transfers This Spring

Reducing hospital readmissions is one of the toughest challenges in post-acute healthcare. This spring, new data from The Moxie Health Group showed signs that more patients across facilities supported by The Moxie Health Group are staying stable, recovering in place, and avoiding a return trip to the hospital.
An analysis from The Moxie Health Group’s RTH Analytics Division showed encouraging improvement across thousands of patient encounters in post-acute and long-term care settings. One of the clearest signs of progress came from a 24-facility network analysis tracking more than 3,300 patients during the month of April.

The report found an overall return-to-hospital rate of 11.80%, down from 12.14% in March. In real terms, that means 399 hospital readmissions among 3,380 patients seen during the month, an improvement after rates climbed earlier this spring.
The reports also highlighted some of the most common reasons patients required hospital transfers, including respiratory distress, abnormal lab results, altered mental status, chest pain, post-fall evaluations, and behavioral symptoms.
Reducing avoidable hospital readmissions remains one of the biggest challenges in post-acute healthcare, particularly as skilled nursing facilities continue caring for patients with increasingly complex medical needs.
The latest spring numbers suggest that early clinical oversight, ongoing provider involvement, and closer patient monitoring may be helping facilities identify changes in condition sooner and stabilize patients before a hospital transfer becomes necessary.
